MEMBERSHIP APPLICATION FORM

"OPERATION RE-ENLIST"

The 174th AHC Association membership campaign, code name "Operation Re-Enlist" has
been initiated. Attached is the current membership form that can be printed, completed
and mailed. The form can be used to become a new member and/or renew an existing
membership. Membership is available to all those who served in the 174th AHC or her
detachments (409th Maintenance, 133 Ordinance, 452nd Signal, or 756th Medical).
